Advance Medical Directive

If you could no longer speak for yourself, what care would you want? This lets you decide in advance, on your own terms, not someone guessing.

Prepared under the Advance Medical Directives Ordinance (Cap. 651)A doctor and a witness at signing

What you get

Your decision, made in advance.

Your AMD, in legally prescribed form

The wizard walks you through the decisions, one at a time. You specify which treatments you would refuse, and under what circumstances. No medical degree required. You review every word before anything is final.

Finalised by Patricia Ho and Associates

An AMD must be signed in the presence of a registered medical practitioner and one other adult witness. You arrange your own doctor. They confirm you understand your decisions and sign together with you and your witness. Then Patricia Ho and Associates reviews and finalises the signed document. One review. No registration. Done.

You can change your mind anytime

An AMD can be revoked at any time while you have mental capacity. Verbally. In writing. By destroying the document. No one can hold you to a decision you no longer want. You are always in control.

Your family does not have to guess

The hardest medical decisions fall on families who do not know what you would have wanted. Your AMD answers the questions before they are asked. It is a gift to the people who would otherwise have to decide for you.

How it works

From form to finalised directive, in four steps.

1

Answer a few questions

Your answers fill the prescribed Cap. 651 form. Takes about 10 minutes. Nothing is final until you say so.

2

Complete your payment

HKD 3,480. Discount available when bundled with EPOA or Will. Doctor's fee paid separately.

3

Patricia Ho and Associates reviews

Patricia Ho and Associates reviews your form for completeness and compliance. This is their one touch, review only, before you take the form to your doctor.

4

Sign with your doctor and a witness

Print your form. Visit your doctor with a witness. The doctor confirms you understand your decisions and, if verified, signs. You keep the wet-ink original. Done.

Why an AMD matters

This conversation is easier now than later.

Without an AMD, a doctor must treat you, even if that treatment only prolongs your suffering. Your family may be asked to decide. They may not agree. They may carry the weight of that decision forever. An AMD removes the burden. It is your voice, recorded in advance, when you can still speak clearly.

Without an AMD

A doctor must keep treating you, and your family is left to decide what you would have wanted. They may not agree, and they may carry it forever.

With an AMD

Your decision is already made, in your own words. The burden is lifted from the people you care about, and your wishes are respected.

Now in effect in Hong KongThe Advance Medical Directives Ordinance (Cap. 651) takes effect from 31 July 2026. Your AMD is prepared and executed under it.

Common questions

Everything you need to know.

No. An AMD is a refusal of treatment. It is not a request to die. It is a legal right in Hong Kong under Cap. 651 to say, in advance, that you do not want certain medical interventions. Euthanasia remains illegal in Hong Kong. This is not that.
Life-sustaining treatment. Artificial ventilation. Cardiopulmonary resuscitation. Artificial nutrition and hydration. Antibiotics for a life-threatening infection. You choose which ones, and under what conditions: terminal illness, persistent vegetative state, or irreversible coma.
Your AMD takes priority. Under Cap. 651, a valid AMD must be respected by medical professionals regardless of family opinion. It is a legal document, not a suggestion. That said, we strongly recommend discussing your decisions with your family before you sign.
Yes. At any time, as long as you have mental capacity. You can revoke your AMD verbally, in writing, or by destroying the document. You are never locked in.
They cover different things. An EPOA covers property and financial decisions. An AMD covers medical treatment decisions. Many people have both. Add an EPOA for HKD 8,100 when bought with your AMD (save HKD 900). Or get The Peace of Mind Plan, Will + EPOA + AMD, for HKD 16,500 (save HKD 1,960).
Ambulance and paramedic services are not bound by an AMD. In an emergency, paramedics will provide life-sustaining treatment regardless of your directive. This is standard practice in Hong Kong. Once you reach the hospital, your AMD takes effect and must be respected.
No. Unlike an EPOA (which must be registered with the High Court), an AMD does not need to be registered. You keep the signed original. We recommend giving a copy to your doctor, your family, and your hospital.

The hardest question your family will ever face. Answer it now.

One form. HKD 3,480. Your voice, recorded. Your family, spared.

Executed under Cap. 651 with a registered medical practitioner of your choosing, reviewed and finalised by Patricia Ho and Associates. Doctor's fee paid separately.
